Home and Community Care (HACC)Eight Home and Community Care (HACC) Services for frail, older people, people with a disability and their carers are under the auspices of Great Lakes Council. The HACC Program is jointly funded through the Federal Government (60%) and State Government (40%) and administered by the NSW Department of Ageing, Disability and Home Care (DADHC). The HACC Program funds a range of services that assist and support frail, older people and people with a disability to continue living at home and accessing their community as independently as possible, thereby preventing inappropriate or premature placement into residential care. The HACC Program also funds services that support carers in their caring role. Many of the HACC Services rely on dedicated volunteers to provide assistance and support to clients and their carers. The following eight HACC Services are under the auspices of Great Lakes Council:
The role of Council's HACC Services Co-ordinator is twofold. Firstly, the position provides information to Great Lakes Council and other groups or persons in the community on aged care and disability services in the area. Secondly, the HACC Services Co-ordinator supports the Team Leaders of the respective HACC Services which are under the auspices of Council in the areas of policy development and service delivery standards. Great Lakes Centre-Based Respite Service - Published: 13 Feb 2009Aims to provide the person with memory loss who lives by themself or with a carer with socialisation, meals and specially designed activities which will take into account each individual's needs. The service is also aimed to give respite to carers who may like to spend the day relaxing, shopping or spending some time doing a favourite pastime. more ..
